My Program Service Status is “Awaiting ESA.” I no longer want to wait for my paper signature agreement to be processed or do not want to mail my paper signature agreement. How do I submit an electronic signature agreement?

  1. Log into CDX

  2. Click on the My Profile tab.

  3. Scroll down to Program Services and select Modify Program Services

  4. Select the appropriate Program Service and Request New Role.

  5. Select the Permittee (signature) role from the drop down and click on Add Selected Role 5.png
     
  6. Enter your Job Title and select Next

  7. Read the signature agreement and select Sign Electronically
